Either mail or deliver the signed form
to CTIS Operations in PWRS 218 on the
second floor of the Powers building between
7:30am and 7:30pm Monday through Friday.
Please Note: Internet mail
will be automatically forwarded to your
Outlook account. Student Employees
will be required to obtain a supervisor's
signature prior to submitting their account
request form.
