or Microsoft Excel spreadsheet format. For most of these documents you will need the Adobe Acrobat Reader to view and print these forms. If you do not already have Adobe Acrobat Reader, you can obtain it by clicking here. Scheduling and Event Form
This form is used to reserve a room for meetings, events, etc. Special set-up requires 10 day advance notice.

Data Sheet/Signature
Authorization Form
This form shows a list of the officers and members in the organization. Must be updated every
school year and when there are changes.
To download this file, click the above
link, and select the save option. Save it to your desktop. Fill out the
forms as necessary and when you are ready to print, Go to File, Print, then
select the entire workbook option. Click ok. Save the file again, then Email
to student life.
- Contract
The contract is required when making an agreement for someone providing services,
whether for a stipend or on a voluntary basis, for an event.

Website Authoring Request
This form enables you to request permission to make changes to your student organization
website.

Alcohol Policy Rider
This form is required for events stating no alcohol or illegal substances will be provided at university approved events.

This form must be completed and submitted for all Chartwell's catering
orders.

Cash Count Report
This form is being
provided for documenting revenue from sales for profit.

This form enables your organization to update information about your organization in various publications.

Event Audit Form
This form is needed to track fundraiser sales.

Sales Control Worksheet
This form is used
to track inventory and sales for products at student organization events.

Leadership Funds Application
This form is required to apply for leadership funds from the Student Government Association.
Only non-selective organizations may apply.

Merchandise Sales
Report
This form is
provided for organizations to document sales revenue and merchandise
inventories.

This form is required to post a message on the electronic message board located on 173rd Street.

Agreement - Member
This form must be
signed by any member receiving a key to the organization's office.

This form is used to record money collected for participation in an event
and should be completed and maintained in the Treasurers Financial Record
Book.

This form is used to record dues collected and should be completed and
maintained in the Treasurers Financial Record Book.

This form is used when a security officer(s) is needed for an on campus event.
