Student Organization Forms

The forms below are either in the Adobe Acrobat Portable Document Format (PDF), or Microsoft Excel spreadsheet format. For most of these documents you will need the Adobe Acrobat Reader to view and print these forms. If you do not already have Adobe Acrobat Reader, you can obtain it by clicking here. After you have installed the Adobe Acrobat Reader, return to this page to view and print the form(s) you wish to use.

Most Frequently Used Forms

  • Scheduling and Event Form
    This form is used to reserve a room for meetings, events, etc. Special set-up requires 10 day advance notice. You may also view an available AV supplies list here.
  • Data Sheet/Signature Authorization Form
    This form shows a list of the officers and members in the organization. Must be updated every school year and when there are changes.

    To download this file, click the above link, and select the save option. Save it to your desktop. Fill out the forms as necessary and when you are ready to print, Go to File, Print, then select the entire workbook option. Click ok. Save the file again, then Email to student life.

  • Contract
    The contract is required when making an agreement for someone providing services, whether for a stipend or on a voluntary basis, for an event.
  • Website Authoring Request
    This form enables you to request permission to make changes to your student organization website.

All Other Student Activities Forms

  • Alcohol Policy Rider
    This form is required for events stating no alcohol or illegal substances will be provided at university approved events.
  • Catering Order/Event
    This form must be completed and submitted for all Chartwell's catering orders.
  • Cash Count Report
    This form is being provided for documenting revenue from sales for profit.
  • Constitution
    This is a sample constitution to guide organizations with the development of their own constitution.
  • Description Profile
    This form enables your organization to update information about your organization in various publications.
  • Event Audit Form
    This form is needed to track fundraiser sales.
  • Inventory and Sales Control Worksheet
    This form is used to track inventory and sales for products at student organization events.
  • Leadership Funds Application
    This form is required to apply for leadership funds from the Student Government Association. Only non-selective organizations may apply.
  • Merchandise Sales Report
    This form is provided for organizations to document sales revenue and merchandise inventories.
  • Message Board - 173rd Street Electronic
    This form is required to post a message on the electronic message board located on 173rd Street.
  • Office Space Agreement - Member
    This form must be signed by any member receiving a key to the organization's office.
  • Office Space Application
    This form is to apply for student organization office space.
  • Participant Roster
    This form is used to record money collected for participation in an event and should be completed and maintained in the Treasurers Financial Record Book.
  • Record of Dues Roster
    This form is used to record dues collected and should be completed and maintained in the Treasurers Financial Record Book.
  • Security Form
    This form is used when a security officer(s) is needed for an on campus event.
  • Start-up Application
    This is an application to obtain start-up funds for a new non-selective organization.
  • Start-up Guidelines
    These are the guidelines to obtain start-up funds for a new non-selective organization.
  • Supporter Agreement
    This contract is required when sponsoring an agency for a fund raiser
  • Ticket Sales Report
    This form is provided for organizations to document ticket sales.
  • University Village Room Registration
    This form is used to reserve the University Village Room for events.
