NSF recognizes the importance of helping under-represented minority students to continue in their career in the STEM programs and strongly encourages students who belong to under-represented minority groups to apply for LSAMP.

A complete application includes a completed information sheet with a choice of research advisor indicated and two letters of reference. The letters of reference should be filled out on the website application by the two people requested by the student. One of the letters of recommendation should be from your potential research advisor. Submit your completed information sheet by April 23rd, 2008

Career or Educational Plans after Graduation: (Because the goal of the program is to encourage student students to remain in STEM fields for their academic program and for their career, plans for going to a STEM graduate program or hopes to work in a STEM career are given further consideration.)
Honors or Awards: (List any awards whether they are from a school or other organization. Awards show the efforts put forth by the applicants and their potential for success)
Have you any experience in a science research program? If yes, please give type and description of the research? (It is not necessary to have any previous science research experience, but it would be helpful to describe any research experience you have had and state if it was successful or beneficial.)
How do you expect to benefit from participation in the Indiana-LSAMP Summer 2008 Undergraduate Research Fellowship Program? (Describe the benefits that you believe you will receive from being in this program. Besides learning the knowledge of your research, how do you think being involved in a research program could help you? What do you think you will learn? Does the financial award of $2,000 enable you to dedicate more time to your academics? Answer questions such as this in your explanation.)

(You should have a suggestion of an advisor from PUC, either faculty or staff. You should have already talked to this person to make sure that you agree that they will be your advisor if you receive the LSAMP. At this point you do not have to know of the specific project you will work on, but know who you will work with. This person will have to be one of the two people that send in a recommendation letter for you. )
