Alumni Questionnaire

This questionnaire is to be completed by the Alumni of the Purdue University Upward Bound Program. Upward Bound has been an integral part of Purdue University since 1966. In this time period, numerous students have graduated from college and continue making significant contributions in society. This page is dedicated to these Alumni for making their dreams come true and helping others along the way.

The purpose of the questionnaire is to identify the progress of Alumni of this Upward Bound program. This information will form part of the federally mandated tracking of TRIO Alumni. Although this questionnaire type is requested of all Alumni who planned to graduate, or graduated, from high school since 1996, we welcome any TRIO Alumni to complete and submit this item to Upward Bound.

Upon receipt, information will be held collectively as statistical data and all personal information will be filed confidentially. This information will be collected by Purdue University Calumet Upward Bound office, which is required to collect and maintain information on its' alumni until six years after their graduation from high school or until their graduation from a four year university's baccalaureate degree program (whichever comes first). Survey results will only be used for reporting alumni data to the U.S. Department of Education which has funded this Upward Bound program since 1966.
